In compliance with Texas House Bill 4224 (adding Section 181.105 to the Texas Health & Safety Code), we are providing the following information prominently on our website to ensure consumers are informed of their rights and how to access their health care records and related resources.

How to Request Your Health Care Records

Consumers have the right to access and obtain copies of their health care records. To request your records:​

• Please include your full name, date of birth, contact information, and a clear description of the records you are requesting to: LaToya@lcscac.com.

• We will process your request in accordance with applicable state and federal laws (including HIPAA).
